Sycamore Health provides a range of treatment options to assist with independent living for those supported with a home care package. 

What are Home Care Packages (HCP)?

A home care package is an in-home support program provided to people as they age by the Australian Government. If you are over the age of 65 and have complex ageing related care needs, you may be eligible. A Home Care Package provides assistance to help you live independently and safely in your own home for as long as is appropriate to do so.

Support might include nursing, alterations to your home to improve safety, assistance with maintenance and cleaning, provision of mobility and communication aids, continence support, or support with bathing and hygiene. A home care package also includes Allied Health Support such as Physiotherapy or Exercise Physiology should you require it.

How do we support people with a Home Care Package

We provide education to help you gain a deeper understanding of some of the impacts of ageing and discussion on how to best tackle problems which may arise, with you ultimately choosing what options are best suitable for your circumstances and context.

How do I access a Home Care Package?

In order to access a home care package, you will need to undergo an assessment from the Aged Care Assessment Team (ACAT). Once assessed, you will receive a letter detailing your eligibility, services you are eligible to receive, evidence for the decision, and your support plan. There are 4 levels to support under a HCP and you will be provided support aligned with the level of care you require as determined by your assessment.


Once you have a HCP, your Home Care provider will be able to help you connect with service providers whom meet your needs.
